Is there a way to enter a LONG list of replacements?

I'm trying to rename some files that contain two variable values - one for the person they relate to and a code for what the specific item is. The code is a ~30-character string of letters and numbers. I want to translate each code into human-readable text (different for each code, but the codes have consistent translations).
I know you can do OR replacements with |, but there are potentially ~150 of these codes that could show up so that's not going to work. Creating a file like rename-pairs would be doable, but I can't hard code the whole file name because the PERSON value changes for each file.
Is there an approach that would work for this? I could swear I've done something similar in the past, but I can't reproduce it now.
